Take a look to simple example of this SharePoint survey web-part usage. We will create standard SharePoint survey list with a couple of questions and then place the survey form on site home page using SurveyUP web-part. In our demonstration we will use SharePoint Foundation 2010.

  • Open Site Actions menu and choose More Option... menu item.
  • Choose template Survey in the Create dialog, enter new survey name (for example Satisfaction survey), and click Create button.
  • You will see New Question wizard after creation the survey. Create some questions for the survey:
    • What method did you use to contact customer service?
    • How satisfied are you with the time it took customer service to answer your question or resolve your issue?
    • Overall, how satisfied were you with the way your question or problem was resolved?
  • Click Finish button.
  • Open site home page.
  • Click Page tab in Ribbon, press Edit Page button:

  • Click Insert tab in Ribbon, press Web Part button:

  • Choose Speals category, then click on SurveyUP for SharePoint web part and press Add button:

  • Choose Edit Web Part item from web-part menu:

  • Enter Satisfaction survey list URL to Survey Address field. In our case it will be http://myserver/sites/contoso/Lists/Satisfactory%20survey/. Press OK button:

  • SurveyUP web-part shows survey questions immediately after specifying of survey list URL:

  • Fill survey form and click Submit link. You will see the survey results:

Using Edit Web Part item from web-part menu, you are able to setup survey results appearance and configure other available options.
